The original Worker's Compensation Application form completed by the horsemen can be scanned into a pdf image, and loaded into the system for paper-less storage. After the forms have been scanned and combined into one pdf file, following the procedures below to upload the file into the database.
Press the Upload button to begin the process. A screen will appear for you to locate the pdf image.
Press the Browse button to open a dialog box displaying the directory structure of your network drives.
Navigate through the directory structure to select the file to be uploaded. Select the file and press the 'Open' button. The 'Cancel' button will stop and exit the upload process.
After the file has been selected, press the 'Upload' button to incorporate the file into the database.
Once the file has been loaded, it can then but viewed by pressing the 'View Image' button.
If at a later date, the horsemen adds new employees, follow the same steps to upload a new revision of the application. Note: Only one pdf file will be maintained for each horsemen's account. When a new pdf file is uploaded, the first one will be replaced by the new version.
